Intermittent P2263 P0486 P0402 Restricted Performance TDV6

Trying to help a friend out with this.
2010 MY, no DPF from new!!

The original fault information is a little bit sketchy, but it was intermittent restricted performance.

It ended up having 2 new EGR valves fitted. These threw up the same EGR code and I believe were changed again. I don't think they were genuine. The owner replaced the MAFs.

I had a look at the car (am used to working on my Jag XF-same diesel engine) and kept getting the water in fuel fault, although the sensor had recently been replaced. I fitted a complete new fuel filter and that fault has gone away.
I have cleaned the MAP sensor.

However, on the odd occasion, usually under light load (slow moving traffic) it judders as if running out of fuel and occasionally, the restricted performance light comes on (although it never did it to me).
I have an Icarsoft diagnostic kit, and logged some live data, but the sample rate was quite slow and didn't pick any obvious issues up.
I get the codes P2263, P0486 & P0402
I hooked SDD up to it and got no more information (although I struggle with using SDD).

Flooring the throttle clears the issue.

I replaced the throttle valve intake Oring and did a pressure test and found no obvious leaks (Smoke test would be better).

Any ideas? On the jag, the TSV can give similar issues, but I haven't checked it on the D4 yet as the guy lives about 30 miles away and I don't want to pinch the car off him unless I have a few things to try out.
