Yep, new fuel rails seems to have solved it. The garage said that the high pressure fuel pump may also be on its way out and, in how ever many months since the rails were replaced, I had one error saying low fuel pressure but the car didn't go into limp mode, just flashed up an error and restart solved it.
The garage, a local LR specialist, say they had a lot of cars last year with the same problem. About half her just fuel rails and the other half fuel rails + high pressure fuel pump. Has nothing to do with the age of the car, they recon the quality of diesel changed in the area.

Yes - new fuel rails and then, after about a year, a new high pressure fuel pump. Done about 40,000 miles since these were replaced and the issue has not come back.
Now the gearbox and turbo are likely on their way out (car is on 224,000 miles) and not sire whether to replace or upgrade to Disco 4 or 5.
