Easiliy done. It's because the notification email contains the PM message. You read the emailed PM and reply via email (to Martin). Then you forget that you did it wrong and thought you'd replied with a PM.

No mate... when you get an email with the PM's contents, people forget to delete the read PM in their forum inbox
Even with the pop up telling you there's a PM, I think some tend to dismiss it thinking they've read it already. But the forum thinks otherwise and its still listed as undelivered.
