I thing the big amount of white water behind the ship is an indicator that the engines didn’t fail, control system failure maybe.

Looks like the engines are in reverse and the rudder hard over. Maybe someone just messed up earlier in the manoeuvre and they couldn't slow it down in time.
Big insurance claim, however, including an injury to a crane operator.

Watch how the crane collapses. It's either bloody good design from a "what if the worst should happen" perspective OR proof there is a God. The injured operator may be inclined to go with option 2..

looks like whoever was steering got it badly wrong, as in having the rudder hard over (thinking that would steer them away from the jetty) it has the opposite effect in pushing the rear of the ship over. He should have steered the front towards the jetty for a while to bring the back out.
