Mine was stuck in this mode since I flashed the CCF to add a D4 module, got it back working by trying everything on here.
Next the mic was not working so I had to remove it to add new wiring. Decided to unpair the phone for safety which was a bad move as it went back to PM Doh!
Tried everything but nothing worked and gave up for the night and regrouped for the morning attempt!
Next day I put the key in the ignition and lo and behold it was working fine! I assume that it is self learning about the status of the mute wire or MOST connection etc? Series 1 V8

Had to pull the fuse box to fix a leaky drain behind it and now back to Phone Mode but more resilient to fixing than before!
As soon as the battery is removed or power it goes back to Phone Mode so that is why the previous post relate to the battery work. Series 1 V8

Now I'm in a two week success. I tried flashing the telephone ECU and it is working. Don't know what will happen if touching again the Audio Front Unit. The CDs (anacronic) doesn't get inside, and I am sure that is mechanical. May be have to try flashing de Audio ECU first.

Phone mode stuck as well. Ignition off, removed fuse 52 (bluetooth module TMC fuse) from central junction box behind glove compartment, ignition on, headunit on music playing, 52 fuse back in and all works ok. The cause was dripping water from map lights on to headunit.
So far so good

This did not work for me.
Ignition on, Off, pull the radio, unplug most, ignition on = everything is fine but no sound, can’t Even adjust the volume.
Most in again, phone mode straight back.
Help?

Been many times in phone mode, and back again to that nice sound. The only way I could manage to bring it back was charging the battery, all night, and then playing with the connector at the back, in and out.
But the last time, two months ago, I saw the light at the end of the tunnel. After charging the battery y began playing the reset button in my Nancocom, and after many times she came back.
I hope this can help you. The point always is battery full charged

Do a hard reset, works every time, cause of it is low battery.

Sorry to be a pain, I'm a complete novice in this. What's a hard reset? And how do I do that? Unit stuck in phone mode yesterday.

Disconnect battery for 15 mins.
Alternatively remove a fuse from behind glovebox, may work

You need to remove both leads from the battery and touch them together. This removes all power from the system and causes all the electronics to start up from there base programs.

Gone to start the car this morning and it seems I have a flat battery, so looks like it would be that, that's caused my issue

Check the alternator output before fitting a new battery they have a habbit of shorting internally and will damage the new battery if failed

Thank you all for your posts, these were all helpful
Ultimately what worked for me (with truck powered off, but never waiting for radio to fully power down)was pulling the #52 fuse first.
This only halfway worked.
So then with the fuse still out (permanently now) I also removed the little blue holder clip between the two MOST lines in back of the stereo. Then removed the drivers side fiber optic cable, and reconnected the passenger side one in the clip and back into the stereo. Power the truck back on, and used my cell phone flash light pointing it at the fiber optic clip where I removed the line, tricked head unit that it had its feedback from the phone module, but went right to radio, because phone module is powered down.
Been good ever since! Flashlight just forces stereo out of phone mode because there’s no communication coming from phone module, just getting the stereo to think it’s getting feedback. Or something like that, or I’m just lucky as heck. Either way, cheers!

Hopefully you've come to this post if you are having this same problem as it's much easier and faster than removing the head unit and all that b.s.
Here's what fixed my problem:
Ignition off (key removed)
Remove fuses 52 and 53 from behind the glove compartment. Wait a minute.
Plug fuses back in - 53 first, then 52
