Think you will find the tricked up D4 ambulances are all part of the HART teams, trained in other things apart from first aid, bit like the equivalent to the fire service USAR teams. MY17 D5 1st Edition Namib Orange

I can only say about the ones in the south East, it's a mix Really in so much that we have stationed round D4's for use on core duties and HART have there's stationed in two locations with the teams. Very Happy Karl
