Another vote for www.aprr.fr which is a really easy site to use and company to deal with.
Just one tip, when it asks you for the French area code, you need to enter five zeros (00000) instead of your post code... Simply make sure your UK postcode goes int one of the othe address lines and 'voila' your tag will be brought to your door for signed-for delivery in about three days.
Other thoughts...
A standard D4 will fit under the 2m height limits on the tele-Peage lines...
A standard D4 will also go through the 30kmph zone at 27kmph and will almost, but not quite, take the barrier with it! I am not always a good passenger and had visions of explaining to Monsieur le Flic why we were dragging 2m of red and white ally barrier up the road with us til my dear wife explained that she HAD slowed for the toll...
Also, when planning your route, check on www.viamichelin.com as this will give standard toll fees for the route.D4 Commercial - 65 Plate, 2016MY

Lots of website explaining how to apply, French is optional. It's easypeasy, even I managed it and I failed my French O Level!!
And it's the same type of tag as the M6Toll so I just use the same mount.

Worth it just to beat the queues. Plus lots of the minor exits are now totally unmanned, no trying to feed the credit card into the slot in the pouring rain and howling wind. Waaaaaaaaaay more civilised and a life saver if you're in BillyNoMates mode.
